ELIGIBILITY
Legend Home Healthcare, Inc. admits

following conditions are required:
The prospective patient must be
homebound;
He/she must be under the care of a licensed Physician who
determines the patient's need for skilled medical care at home;
The
needed service must be reasonable and necessary according to the patient's pay
source (see payment options);
There must be a reasonable expectation
that the medical need of the patient can be met adequately in his/her home.
